Each year NAC Roster Artists complete this form to let us know if you wish to continue with the Artists in Schools/Communities (AiS/C) and/or the Creative Aging Artist Program (CAAP) programs. This is also an opportunity to request changes/updates to your NAC website listing and to provide your current full mailing address for NAC records. Your personal address will never be published.
